I was looking at alot of cracking guides etc and most said that I should use unique passwords because most user:user combos won't be too good not sure if it's true or not. I'll use user:user next. Is their anyway I can avoid getting accounts that have password change failure? that's my main problem I don't like getting those lol

Are you using a shared LoL config or a private one?

The thing is that many people are using the same.

Whoever told you user:user is bad was right ...

I don't think there is a easy option to prevent password change failure"

except for maybe checking it after cracking it sorry.

Search for tutorials how to build a sentry config. It is pretty easy to make your own. Shared configs are not good at all
